Appointment Text Message Terms
Crown Valley Imaging, LLC (CVI) sends appointment-related text messages only to people who have agreed to receive them. This program covers appointment confirmations, reminders, schedule changes, and replies to messages you send us. We do not send marketing or promotional texts through this program.
Message frequency and rates
Message frequency varies with your appointments, scheduling changes and replies. Message and data rates may apply.
Consent and care
Your opt-in may be recorded on a signed form, through a website opt-in when one is offered, or through a verbal request you make to staff. Consent is optional and is not a condition of receiving care. You can opt out at any time by replying STOP. If you opt out, you can still contact us by phone for appointment information.

Privacy
CVI does not sell or share your mobile number or SMS opt-in or consent data with third parties or affiliates for their marketing or promotional purposes. Service providers may handle the number and message information only as needed to deliver or support this program for CVI; they may not use it for their own marketing.
